COD (like any payment method) only appears where a shipping rate exists. If a country you want isn't covered by a zone with your linked COD rate, add it there too: the ACOD rule controls visibility, the zone controls existence.

Either the country is missing from your blocklist (or present in a zone your allowlist rule doesn't cover), or a second app's payment customization is also active. Testing mode plus a checkout with that country's address pinpoints it quickly.
